New Patient Registration

Cullybackey_logoTo register as a patient of the Practice, phone or ask at reception for details. Please see the next tab for details of our Practice boundary.  We would encourage patients to register with the practice nearest to their home address.

All new patients are invited to attend the Practice Nurse for a consultation and short registration examination.  This is to introduce you to the Practice and also because it can take considerable time for us to receive your medical records.

We will need your NHS Medical Card, and you should also bring a list of any medications you are taking (whether prescribed or bought over-the-counter), and a urine sample in a clean container.  You will also be required to complete a registration form.  If you do not have a Medical Card, please bring some form of photographic ID.  Please also bring proof of address.
